Do you have a case on the phone that covers the microphone on the back next to the camera lens? Or a flip-open case that you hold against the back of the phone when on a call? In either situation the back mic must be uncovered when you are on a call.
Beyond that, you should test all of the microphones, and if any aren't working make a genius bar appointment to have the phone evaluated.

Do people hear you well if you use the headphones?
If so, then it's a microphone problem for sure, not coverage or software.
