I would suggest you reset your Watch. Press the crown and side button in until the Apple logo appears. Still issues unpair and repair it. Check your updated to 5.2 and your iPhone is running iOS 12.2

I have found the floor counter to not be so accurate when it is very wet (humid) in the current environment, or when the watch is wet.
One thing to remember is that the watch counts floors based off of the elevation you have walked/ran/cycled. So walking up a hill can count towards going up a "floor" or two.

While the watch does determine number of floors based on altitude, it's a little more complicated than that.The watch also takes into account what sort of motion you're engaged in. Riding a bike or riding in a car up a hill will not result in the phone thinking you've climbed stairs. Even changing altitude while hiking doesn't tend to result in flights of stairs climbed being recorded

Ok I would suggest you call Apple Store make appointment for them to look at the Watch. As there must be a some wrong. As you have AW 4 it will be under warranty.
