Boot into Recovery (Command R) and from the dropdown menu: Utilities>  Disk Utility> run the First Aid on your Macintosh HD. If errors are found and repaired, run again until no errors reported.

Recovery http://support.apple.com/kb/HT4718

Reboot and test

You can Download macOS Mojave 10.14.4 Combo Update - Apple Support https://support.apple.com/kb/DL1996?locale=pt_BR

this is known to sort anomalies not easily explained away.

If no resolve you can reinstall the entire macOS on top of your existing macOS from the Recovery—this leaves your user files/data untouched.

Always advised to have a current backup.
