Control-v Paste does not always work

Control-v does sometimes does not work, like when I try to paste a copied name when creating a "new" or "save-as" document. It also does not work intermittently in apps and other MacOS scenarios. But the right-click or drop-down menu "Paste" command works fine in each case.

I spoke with Apple Support and a Senior Advisor said that it happens on his iMac as well, but had no solution.
