Why do I see QR code on tv screen after pluging in lightning av

I am trying to hook the lightning device to my iphone and tv and can't get the phone screen to come up. We only see the square qr code box

I am trying to read a QR code in an app on my phone. When I turn on the rear facing camera the QR code doesn't show up. I only see myself. How do I get the QR code to show? How do I read a QR code residing in an app on my iPhone? I can turn on the rear facing camera as in the instructions, but I see myself, not the QR code.

Watching videos on tv from iphone I bought a HDMI so I can watch videos on my tv, it worked just fine but now all I get is a QR Code ( the white box ) any suggestions?
