I have not been able to get a straight answer as to group competitions on the apple watch. Is it possible for 4 people to compete in a group competition with 1 winner after the week? I don't want to compete separately.

No, that is not possible.
However, there are third party apps that may do what you want. There used to be a lot of them. The only one I can locate now is Activity Club.
You can compete with up to 40 of your friends. But only can see one at a time for the competition.
