how to unlock a locked apple ID while forgetting many details

My Apple ID is locked, and I don’t know my security question answer, my phone number, my postal code and even my address. As I said, I don’t know any of my Apple ID details. I only know my password, my birthdate and nothing else. Unfortunately, my Find my iPhone is on and my phone will be locked if I reset it. What can I do? Are there any ways to turn off Find My iPhone so that I can reset my iPhone and use a new Apple ID?

From what you say it's possible you won't be aqble to unlock it. See if this page helps at all:

If not, all you can do is contact Support: https://getsupport.apple.com ; however they are going to need sufficient eivdence to prove you are the owner of the ID.
