slow imac, bbod persists after reformat/clean install

I have a late 2014 27-inch Retina 5K iMac running Mojave 10.14.5.

It suddenly became very slow after failure to wake up requiring a hard reboot.

I tried multiple options with no improvement.

Ultimately I reformatted the HD and reinstalled OS X.

It still is beach balling and lagging intermittently, alternating fast and then very slow/freezing.

I have run disk utility first aid, say "repair successful".

No change.

etrecheck report to follow (says too many characters)

Please help! Is the HD failing?

Thanks for any help!
