Yes, If you are storing passwords on Settings in accounts & passwords linked to your Apple ID
Once you open your iPhone with this Apple ID you will find all passwords on iPhone to & you can update them at any of the 2 devices since this feature is linked to iCloud
While if you are using 3rd party app from the store, you have 2 options if this app allows the passwords transfer of course
either the app stores the password on iCloud then you just get the app on iPhone & link with Apple ID & same app account & you find your passwords, or the app is using different cloud service to provide you passwords linked to your app account
