Apple Watch 4 speaker issue

I used to be able to make and receive calls ok on my Apple Watch 4 but now I can't hear anyone, even with the volume at maximum. I've done a restart of both the phone and watch but it still persists. How do I fix it?

Unpair your iPhone from a Bluetooth headset, if one is presently paired to it. Restart both your iPhone and your Apple Watch (normally), turning both devices off together, then restarting your iPhone first: Restart your iPhone, iPad, or iPod touch - AppleSupport. Restart your Apple Watch - Apple Support.
