Iphone 6S disabled

Hello. The other day I cracked my iphone screen. It was usable for a couple days but I think the crack actually damaged the touch screen and parts of the screen stopped working entirely. Then the phone started going crazy and would continually try to enter a passcode by itself and kept entering wrong passcodes until the iphone became completely disabled.

I have since fixed the screen but I am unable to enter my passcode. The iphone just opens with 'Iphone is disabled - connect to itunes' and the option for an emergency call. I have tried connecting to my computer and itunes account but when i do the iphone buzzes three times and i am unable to find my phone connected to my computer anywhere. I have connected my other phone to the itunes library and it worked fine so i know the problem isn't the cord - what should i do? How do i connect this disabled phone to itunes?
