how can I fix it and why did it happen it happened out of nowhere

Is something (cover and/or screen protector) interfering with your front camera? (try to remove them).
Shouldn't be this case, and if nothing changes if you restart or force restart your device, the front camera glass could have been damaged. Have your device inspected at an Apple Store or Authorized Service Provider:
