Thanks for your reply as it explains my confusion. I have used Apple Pay in connection with a bank account transfer to a family member. Just recently had some one send me Apple Cash and not sure how to spend it.

Apple Cash does not work with a merchant but Apple Pay does - How to use Apple Pay - Apple Support
you have to transfer it to your bank account and then it becomes available for Apple Pay. Transfer money from Apple Cash to your Visa debit card or bank account - Apple Support
