how do i remove my iPhone from a friends iCloud ?

Your friend can sign in with his Apple ID and password at www.icloud.com and remove your device see this article https://support.apple.com/kb/ph2702?locale=en_US

I have done that but once she signs back in to her iCloud my phone appears on her iCloud again ??
