I just tried to add on an iTunes voucher to my account and it hasn’t gone on but when I do it again it says the voucher has been redeemed

Have you tried logging out and back into the account and seeing if its balance updates ? If it doesn't : http://www.apple.com/support/itunes/ww (click on your country's flag, the country where you live, and which is therefore the country on the account ; click on Contact Support towards the top right ; click on Get Help under 'Talk To Us’)
