I think my girlfriend has hacked my phone and installed a tracking system on it,how do I find out for sure?

You’re using an Apple Device. No one is hacking your phone.
If you still feel a need to secure your account, then use Two-Factor Authentication for your Apple ID. Also, run MalwareBytes for iOS, which is found in the App Store.
Has your girlfriend ever had access to your unlocked phone for more than a few minutes? If so, it's possible (though still not likely). Restore your phone to factory state.
Could your girlfriend have guessed/found out your Apple ID? If so, change your Apple ID password.
Going forward, put a passcode on your phone and don't give it to anyone.
Force the phone into recovery mode and restore iOS using iTunes, then set it up as a new phone.
Then get a new girlfriend.
Not possible to hack iPhones, or any iOS devices, unless it was jailbusted! Most likely follow what other people said, and erase iPhone and set it up as new.
