My iPhone 7 on Verizon running iOS 12.4.1 recently has been taking a long time --like up to 10 seconds--to start ringing on outgoing calls. Sometimes nothing happens. I notice the signal strength indicator goes from full strength to one or two bars and then it starts to ring.

The phone is searching for network and an available channel. This can happen if you are in a very busy area and the nearest tower is overloaded. So the phone will then look for the 2nd-nearest tower, and continue looking until it finds one with an available channel. The further away it has to look, the lower the signal strength.
Please try changing the SIM and restarting the iPhone
