when I use any Bluetooth headset, my iPhone microphone is used instead of the device’s microphone. This remains true across devices and has remained true when I changed from iPhone 6+ to Xs max.

Thanks for that info and for choosing the Apple Support Communities. If I understand correctly, you are unable to use the microphone on a Bluetooth headset when using it with your iPhone. To help with this, see if any of the steps in this Apple resource help:

Since you said this happens on both your current iPhone and happened on your old iPhone, use iTunes on your Mac or PC to backup and restore your iOS software to factory settings, as this process erases the information and settings on your device and installs a fresh copy of the latest iOS software. Temporarily set up your phone as new to test your issues, and then restore your back up and test the issues again.

What your advising makes sense. I may never find out what is wrong, but it is likely to fix the problem. Good enough for me. I have extensive apps loaded on my phone and multiple pages set up, and would hate to lose that customization. Do you or anyone else on this list know of a way to preserve my Springboard set up? Once, I lost the system, and had to rebuild it by hand which took a few mindnumbing hours of moving icons around.
