Hey Siri Voice feedback not working, only working if activated by side button (while using bluetooth)

I've never had this issue, but using Siri in my car I only get audible Siri voice feedback if I activate Siri using the side button. Accessing Siri via "Hey Siri" visually works, and she does whatever i ask her to, but there's no actual Siri Voice or beep. Works fine if I activate her from the side button.

One thing that is interesting, when Siri screen is active, i tap the bluetooth in the lower right hand side, and it shows "iPhone", and my car's name. But my car's name is listed twice. I can't find a way to reset that.

Attempted resolutions:

• Unpaired and re-paired with car
• Made sure Siri volume is all the way from Siri screen (again, problem only presents if activated by "hey siri")
• Removed Siri and Dictation and reset them
• Reset Network Settings
• Hard reset
• Wiped phone and restored from iCloud backup

Thoughts? Anyone else dealing with this? It's super annoying. I rely pretty heavily on Siri while driving. Never seen something like this.
