You need to cancel the subscription by following the steps here: Manage your Apple Music subscription - Apple Support
Then request a refund for it to iTunes Billing : Request a refund for an App Store or iTunes Store purchase - Apple Support
Yes, I do wish to request a refund for non-usage. However, I found no icon, no hyperlink, and an inactive "view purchase" in the report a problem webpage. Can you tell me how I can locate the right place to report this issue? Thank you.

In order to cancel your subscription, all you would need to do is go to Settings>{Your Name}>iTunes & App Store>Subscriptions, select the subscription for Apple Music Family and simply un-check it. Keep in mind you will be able to access the music until the end of the cycle. If you wish to request a refund for non-usage, you would want to visit http://www.reportaproblem.apple.com and sign in with your Apple ID and password. From there you would just locate the charge and select your reason for wanting to end the subscription. Please be aware that having the ability to cancel immediately and receive a refund will be generated by the website.

If you are interested in requesting a refund on demand for non-usage and are unable to do it through that site you would need to do it through Apple iTunes Store and Billing Support agents directly. You can arrange for them to contact you by visiting http://www.apple.com/support
