Have you clicked the words "contact Apple Support" below (from the previously supplied link and told the advisor that you need help canceling the AppleCare plan? They are going to need some information from you.


When you're ready to cancel your AppleCare plan, contact Apple Support and tell the advisor that you need help canceling the AppleCare plan. It may be helpful to have the AppleCare agreemeent number from the Proof of Coverage certificate, the Serial number of the device that the AppleCare plan covers, or your original sales receipt.
