No one here would know. But, it most certainly seems like a scam, being a deal-and-a-half, per se. If it were me, I would just not take part in the survey. And I would just blow it all off, trashing the whole thing.
The old saying goes, “if it sounds too good to be true then it probably is.” I would not respond to the survey nor provide any personal information; it is highly likely that it is a scam!
