iPhone 6, xfinity mobile, blocked calls

This week my wife called my iPhone from her iPhone and left a voice mail. I never received the voice mail or had a missed call on my phone log. I then tried to call my iPhone from my land line and it went straight to voice mail, my number was not available. It just started this week. I searched and found some issues with people making calls, which I could do, but not receiving them, except telemarketers, I could receive their calls. I had changed to xfinity mobile in June and assumed that it was their problem. I spent an hour chatting with them, resetting my phone, cancelling call forwarding if I had set it up, etc. No luck. This morning I thought to check my blocked call list and was shocked to see that I had hundreds of numbers blocked. I scrolled down the list and the last 3 were listed as my wife’s name and her mobile number, our home number, and another number we use. How did this happen? I didn’t block her and didn’t block all those numbers on the list. Is my phone hacked? I know spoofing happens. I had a call once and it said it was from me, now that is spoofing. Any thoughts?
