I believe my iPhone is also hacked due to the performance and the screen time being used

While looking in my iPhone I noticed in the Legal Notices under Legal & Regulatory that both LOINC and RELMA are mentioned also a BLUEmagic. Not to sure if this applies for all apple products as I did my research and seen that there is no connection to apple and LOINC or RELMA. I was hoping I could receive insight on this before reporting software piracy to SIIA. I do believe my iPhone is also hacked due to the performance and the screen time being used when I haven't used the specific app.

I can’t speak for the 1 billion other phones out there, but it is on mine. Along with:

  • ACELP
  • UV22HR
  • BLUEmagic
  • Hirago
  • Linotype
  • Snyder Fonts
  • MPEG
  • HiRISE
  • NIKE
  • QDesign
  • Sorensen
  • IntelliWriter
  • WolframAlpha
  • MLB
  • QRCode

Essentially all of the technologies that Apple licenses from other companies and institutions.

LOINC is a standard for clinical data reporting. RELMA is a way of distributing LOINC data. They have nothing to do with hacking an iPhone, which can't be hacked unless it is jailbroken. And Jailbreaking requires physical possession of the iPhone for 15 to 30 minutes while it is unlocked. So unless you gave it to someone that you don't trust while it was unlocked it has not been hacked.

I see you are connected to VPN, however. Unless it is a trustworthy VPN it could be tracking your usage and data and selling that information. If you are not paying for the VPN service it is almost certainly untrustworthy and you shouldn't be using it if you value your privacy.
