You need to contact your carrier. Only they can supply that information.

Here are a few options for contacting your carrier:

1. Visit one of their stores
2. Go to their website and use their chat support
3. Borrow a phone from a friend or family member to call them
4. Use a phone app on your computer or phone like Skype to call them
5. If you have a User ID and password for their online account management service log in and you will most likely find a way to recover your PUK on your own
6. Get a replacement SIM card
