Currently running 12.4.1 on my iPhone. Running High Sierra (10.13.6) on my iMac.
Will I lose the ability to play my own songs in my iTunes Library with either iOS 13 or OS 15?

No. Both iOS 13 and macOs 10.15 Catalaina retain the ability to play music not bought from iTunes. you can still sync music from Catalina to iOS devices regardless of its provenance.
