ECG requires the following software: 

• The iPhone must have iOS 12.1.1 or later installed
• The Apple Watch must have watchOS 5.1.2 or later installed

you can't do anything

ECG is viewed as medical equipment can can't be activated without government approval

it will work when the government approve it

if it was purchased in another country then where you are located, than that government has to approve it too before it will work
