You can add AppleCare+ to a new phone within 60 days of purchase. The phone must be working at the time you add AppleCare+; Apple will test it remotely if you can’t conveniently take it to a store.

If you already have AppleCare+ that is close to the 2 year expiration you can extend it on a month-to-month basis—>About continuing your AppleCare+ coverage for iPhone, iPad, and Apple Watch - Apple Support
