Providing you have a new qualifying device or purchased a refurbished unit from Apple those steps should work. Open box and refurbished devices from other sellers would not count. If you have verified and it's still not working then contact Apple support.

No problem. The offer started as of September 10 and users have 3 months from purchase to claim the offer. For those who purchased it prior to the Apple TV Plus launch date then they would have their 3 months begin as of the launch, Nov 1st. That would put the expiry at Jan 31st. If you fall into that then unfortunately you would officially no longer be able to claim the offer. However, you can contact support to see if they would grant an exception.

I have this question too. I have just spent a(another) half an hour on the phone to support, who tell me that the 8 TV programmes `I can see on the "Apple TV+" app, for free, are the total number of free programmes available to me with this free year of Apple TV+ but they will add some more in future - REALLY?

Thanks for the info. Brand new phone. When the new iPhones popped out this was purchased. I haven’t had the chance to get the deal. Now that I want to do it, it’s not allowing me to. I guess I’ll call Apple and stay on hold for eons to get this taken care of.
