How can I scan a QR code with my iPhone 7Plus? Apple’s instructions for use camera do not work.

What iOS version are you using on your 7+? It must be version 11 or later.
Is Settings > Camera > Scan QR codes turned on? If it is, turn it off and back on and try again.

The second part of step 2 - i.e., "Your device recognizes the QR code and shows a notification." - never happens.
My device never recognizes what's in the viewfinder as a QR code.
I've tried obscuring other things (e.g., "blocking" the sides of the QR code with a blank sheet of paper), turning up my screen brightness and "tapping" to focus. None of those things work.
I tried @AJ397's suggesting (cycling the QR code setting in Camera) and rebooted my phone. That did not work on the QR code I'm trying to use (from a medical card).
However the code posted just above by @ProustGiulio did work.
It's the latest version - 13.3 ("Your software is up-to-date")
Try this on your computer, and keep your iphone in portrait mode. Can't you see the notification on the top of your camera app?
Perhaps in that medical card the QR code is not printed correctly.
