Configuration profile not installing, creates install loop

I have a number of users who are all on iOS 13 that cannot install the configuration profile for a beta app we have released through App Center.

The users all see the profile available for installation within Settings, opt to install without a problem. However, when they return back to the settings screen, it appears that the profile is still available to be installed.

When they navigate back to Settings/General to view the profile, it does not appear.

We have troubleshot this by restarting the phone, various methods with Safari (clear cookies, cache, all tabs, and killing the app), and tried another browser but Apple restricts Configuration Profiles to be installed exclusively through Safari.

We tried moving forward and installing the app anyway as App Center recognizes the device as provisioned but the app "grays" out around 75% completion on the installation. If you tap the app after this occurs, it says the app failed to install.
