Apple Watch Disconnecting From Activity App

How do I get my watch series 5 to stay connected to my activities app. They were supposedly disconnected earlier this week and I also noted that the bluetooth on my phone was saying that my watch was disconnected from the phone. But I was still receiving phone calls and text messages on my watch. I did all the troubleshooting I could find and nothing worked. Finally, I unpaired and repaired my phone and watch. Today it started acting up again. Not synching with my activity app, not "connected" to my phone by bluetooth, but still getting notifications. How do I fix this?
