Depends, ive had some refunded, and some not. Seems Apple have it embedded in their t&cs that children making purchases by mistake is nothing to do with them, even though they know it’s a big problem. They would rather keep the money.
I've always thought this is why Apple for many years has required the Apple ID password to be inputted at least every 15 minutes. You can of course set it to be required for every purchase. So you might want to change it to that setting. You probably have it set to the "every 15 minutes" setting. Or the child in question knows your password.
Anyway, it is fair to say Apple has been aware of this problem for a long time.
Apps like Angry Birds used to get purchased by the little ones frequently.
