Go to the bottom of the page to “Tell us how we can help” Click the Get support tab. Choose “Disabled Apple ID” and then “Your account has been disabled in the App Store or iTunes alert”—which will allow you to schedule a call with Apple

Your Apple ID is your email address. Do you mean you forgot your email address? Why don’t you just look at an email that was sent to you, and see what address it was sent to?
