after "restart" power button is push, then the option +command+ R; the screen now shows "start up manger" then world spinning slowly and 24 clock beneath it..

It means the computer cannot find the startup volume. It sounds from your description like the HD has died. Please take the computer to your local Apple Authorized Service Provider for a diagnostic and repair. If it is an older Mac, you also may want to consider replacing it.
