Follow these instructions—>If you forgot the passcode for your iPhone, iPad, or iPod touch, or your device is disabled - Apple Support. You will need a SIM card to activate the phone. It does not have to be a currently active SIM card, but it must be for the carrier the phone is locked to if it is SIM-locked.
