Latest settlement for old device throttling

How do I claim my two devices on the new settlement that apple is offering? I have an old iPhone 6 that stopped working due to apples shenanigans and currently an iPhone 6s plus.

I had to change the battery in the first one 3 different times and the screen twice because of flex damage from the battery ballooning up. Had severe issues with the throttling also. Eventually the phone stopped working properly and I started having "phantom touches" on the screen (essentially I would lose control of the phone and the phone would start going into various apps by itself)." Now I have an iPhone 6s Plus. It has similar issues. In a year and a half I have changed the battery and the screen once. I take care of my phones very well and the speed at which these are wearing out is pretty ridiculous.
