A box with the time of day randomnly shows up in the middle of iPad screen

My 91-year-old Dad who lives in a different state from me has an iPad and he's been complaining for over a month that a box with the time of day randomly shows up in the middle of the screen and he doesn't know how to clear it. He says it eventually goes away, but it interferes with whatever he's doing. I've never seen it on my iPad. I finally got him to take a screenshot when it happens. I've have pasted that screen shot below. He was looking at the Clock app and this box that says 7:29 showed up (oddly, it's sideways also.) Does anyone have a clue what's happening?

Unfortunately, it's still a mystery. I walked through every Accessibility setting with my dad over the phone and I can't see what it would be. The only things he has selected are Assistive Touch (which I also use because it's brilliant for volume controls and screenshots) and Larger Text. This silly sideways box showing the time of day keeps showing up on a regular basis. It goes away after a few seconds, but he can't find a way to dismiss it without waiting for it to disappear.
