Refund was approved since child bought stuff on my ipad without permission but never got refunded?

I submitted a refund request since a child/minor in my family got ahold of my ipad and bought stuff on a game and it was approved but i was just notified by my bank that the purchase never got refunded and still went through

iPad mini, iOS 13

I requested a refund from games that my child had spent money on without my permission. It says that the decision has been reached however I can’t see what that decision was? My child was able to purchased items in his games without my permission and he bought quite a lot on 2 separate days. Refund for in app purchases On Jan 30th my handicapped daughter bought approximately $250 worth of in app purchases. We caught it immediately and called Apple. The person we spoke with said he can refund that but I haven’t seen it reflected in my account yet. I set up my daughters phone so she could not do that again but I still haven’t been refunded. Can you tell me why?
