I've closed this website under "Website Using Microphone", cleared my history, and clicked command while I dragged the icon out but this icon still persists. Also, when I click on "Mute" it doesn't do anything.

It looks like this is related to a BlackBoard Collaborate session. Do you have any browsers or a BlackBoard app open on your Mac? If so, quit the apps and see if this helps:
