How can you get a refund for a subscription I never Authorized?

You need to cancel the subscription: See or cancel your subscriptions - Apple Support
Then contact iTunes Billing for a refund: Request a refund for apps or content that you bought from Apple - Apple Support
Note, if you downloaded an app that had a free trial, then you did authorize the subscription. When you download an app with a free trial you agree to it renewing into a paid subscription unless you manually cancel it.
