iPhone 11 Pro - Zoomed In Lock Screen

The Lock Screen on my wife's iPhone 11 Pro occasionally "zooms in" in the sense that the screen figures and buttons on the screen become very large and cannot be accessed because they subsequently off the screen real estate. I am not explaining this very well but it is very frustrating as you have to do a hard reset to get it to work. It is very random, and still happens after the most recent iOS update.

Any ideas what this could be?

Sounds like Zoom. It is in Settings>Accessibility. You can turn it off from there, you can also activate it from the screen. Double-tap 3 fingers to zoom, drag 3 fingers to move around the screen, double-tap 3 fingers and drag to change zoom. You can also check Accessibility>Accessibility Shortcut to see Zoom is turned on there. To access the Accessibility Shortcut, you would quickly press the side button 3 times.
