Can some kind soul advise on how to approach the major and minor issues reported by etrecheck?

Oh dear. That seems very blunt and a bit drastic - do you mean on MacOS or on any system? Is this an accepted opinion across all the Apple Community, and are there any published comparative reports?
Kaspersky and any other Anti-Virus programme is not needed on macs. There are no viruses that attack the mac,
so there is no value in installing such apps, they just take up resources and can flag up system files as being a threat that you then go and delete. The same is true for any mac cleaners or optimisers, they are not needed either and can often degrade the performance of your mac as opposed to their promises of enhancing it.
The mac looks after itself pretty well, if you want to keep on top of adware/ malware then install malwarebytes, there is a free version. https://www.malwarebytes.com/mac/
