Which version of Catalina do you have installed?

Did you create the duplicate while viewing an album with "Image > Duplicate 1 Photo ⌘D"? This will duplicate the version, but not the original image file. The two versions will share the same original file.

The duplicate version may not be added to the same album. That will depend on the version of Catalina. and duplicate may be suppressed in the Days.

To see the duplicate ctrl-click the original and use "Show in All Photos". The duplicate version should appear directly beside the original.
