My wife was using her son’s Apple ID to download app’s. She now has her own Apple ID setup in her phone but in the health app it still shows her son’s picture and information. I can edit his information but I can’t get rid of his picture any advice would be great

oops. You are completely correct. The Health App is a native app and it can't be removed.
You are also more than likely correct in that the iPhone was never prepared correctly for her use as the new user.
She needs to delete the Health App and then download it under her own Apple ID. Apps are forever tied to the Apple ID they were downloaded under.

I wish I could.
I just wonder if the phone was properly cleared down and signed out of Activation lock.
