Why ECG not available in my country (iraq)

I'm sure there is a country named Iraq. But as far as the ECG app is concerned, it must be approved by the government of the country where the Apple Watch is sold And it must be approved by the government of the country where the Apple Watch is being used.

So if ECG is not working in Iraq, then there are governments that have not given their approval for the medical device called ECG on the Apple Watch.

ECG requires government approval.

But in this case it could also be government interference preventing Apple from approaching your government.

But as it is, it is illegal for Apple to offer ECG in any country that has not provided government approval.

Why is there no country of Iraq?

Because the government of Iraq haven't approved it yet and we don't know when or even if it will get approved.
