Apple TV+ App not showing up

I have signed up for the free 1 year with the purchase of my Apple TV. The Apple TV + icon/app is not showing up???

Anyone know why and how I can get the Apple TV + working?

I notice the old style TV app in the upper left corner. Please update to tvOS 13.4.5 if you haven’t already done so.

There is no Apple TV+ app icon. Apple TV+ is one of the “Channels” inside the Apple TV app (simply named ‘TV’ on the Home Screen). This is what the redesigned TV app icon looks like (after tvOS 12.3/tvOS 13):
