PCI problem - Intensity Pro suddenly not working

It's been a long journey to get to the forum post. Here's the problem:

I was using an Intensity Pro ( HDMI output card ) with Final Cut Pro 7 and After Effects CS5. Suddenly my Intensity Pro card gives out. Thinking it might be a driver problem, I upgrade the drivers to the latest version ( 3.7.2 ) but nothing happens. Now thinking it's a hardware problem, I order a new Intensity Pro card. I get it in. Install it. Same thing. Even with the drivers installed, the system can't see the card. So I try it on an identical Mac Pro with the same driver. Same problem. Doesn't show up. So I think, some how, this new card is defective. So I do the RMA and get a new one in. Again, same problem.

I noticed in the System Profiler that the card shows up along with the 2 GPUs, but instead of showing the lane width as 4x, it's shown as 1x, and shows that the drivers aren't installed even though they are. The motherboard even has either 16x or 4x printed in white next to each slot. The Intensity Pro card is installed in slot 4 which has a speed of 4x ( other slots yield the same result ). I'm not sure if the info in the System Profiler only updates when the card can communicate with the driver, but for now, it doesn't.

Everything I've mentioned about the Intensity Pro card is also true for the Decklink HD Extreme. I uninstalled the Intensity drivers, restarted, installed the Decklink drivers and restarted again, with the same result. So I uninstalled the Decklink drivers and replaced them again with the Intensity drivers.

Again, this behavior is the same across two identical Mac Pros ( same GPUs, CPUs, RAM, etc. )

I can't get anything to output of FC or AE if the system can't recognize the card. Plugging other cards into the same slots works ( Fiber Channel, other GPUs, etc. )

My OS is at v10.6.4. I'm out of ideas and I need to get this working again.

Any help would be greatly appreciated.

answers to your question:

1. Yes, both are Mac Pro 4,1

2. Other cards include:
- NVIDIA GTX 285 in slot 1 and an GeForce GT 120 in slot 2
- the official ( sold by Apple ) Fiber Channel Card in slot 3
- slot 4 has the Intensity Pro

3. I haven't tried an SMC reset. I'm not having any of the symptoms described in Apple's documentation:

I will give the SMC reset a shot when I get back to the office tomorrow morning.

HI Grant,

I'm having the same problem. Got the Intesity Pro yesterday and it just wont work. No signal in or out and showing up as a device in any CS5 apps. Installed and reinstalled drivers, card etc etc. Nada! I just wondered if you had any luck before I jump out of a window? System profile info below:
