i can't sign into my account

everytime i try to sing in to my account, error message shows up

on my phone the message is : verification failed,,, the application you have selected does not exist.

and on the apple website .. i enter my account for 5 second and then it sign out by it self

and then it says the session has timed out for your security.

can you help me please
