If it is the microphone while you narrate, then click on the Option next to the record in Command-Shift-5 and select the microphone source.


If using Quicktime Player, then click on the down v next to the record button and select the sound source.


I'm not sure if how to capture the sound of a video you are playing and also trying to record, if that is what you are doing.
